Attached is an employment application. Read over all five pages. Complete the application packet. A resume is not an application. Sign and date pages four and five.
The three Reference letters need to be completed by your current or previous employers. Make sure to inform the person completing the form to include his/her name and a telephone number where I can contact him/her for employment verification.
You will need to obtain a Cincinnati Police background check original with a raise stamped seal (There will be a cost of four dollars to you for this service). You can get the background check at the Justice Center on Sycamore Street, in downtown Cincinnati, Ohio.
Once you have the above paperwork completed call B. Macke at 513 861-8483 to set a time for an interview.
At the time of the interview bring the above completed paperwork, the Police background check, and your driver's license and social security card to be copied.
